We are excited that you have chosen us for your medical care and look forward to meeting you. As a new patient to our practice, we know that you have questions about our policies. So, we've listed them for you below.  You can also save some time by printing out and completing the New Patient Forms in advance of your appointment so we can expedite your first visit.

APPOINTMENTS
If you are unable to keep your scheduled appointment, we ask that you give us at least 24 hours notice ($35 same day cancellation fees).  Otherwise, please arrive 15 mins prior to your appointment for filling out necessary forms.

PRESCRIPTION REFILLS
When you need a prescription refilled, please contact your pharmacy and request for electronic refill request submit to our office .  If you need a paper prescription request, please call our office at least 2-3 days prior to running out. This gives the physician time to review your chart and approve the refill.

We do not refill medications after hours.

PRESCRIPTION PA (Prior Authorization)
Please allow 2 - 5 business days for transaction to process.

OFFICE HOURS
Monday - Thursday: 7:50am - 6:00pm  (Lunch break 12:30 - 1pm daily)  
Friday - Saturday - Sunday:  Closed

We accept most major insurances.  Co-payments; Co-insurance and deductibles are due at the time of service. We accept major credit cards, checks and cash.
